The survey should take no more than 10 minutes to complete and is anonymous. Please think about your use of the Library and Museum Services over the last 12 months when choosing your reply. We rely on input from our customers to improve our offer and this is your opportunity to influence our service.

As a thank you for completing this survey there is an optional prize draw to win one of five £30 M&S vouchers. If you want to enter, complete your contact details when requested. 

RCN staff and their families are not eligible to enter the prize draw.
